Omission Response Trigger Codes

I am working on a task that sends response codes (correct, incorrect, no response) to a parallel port device for EEG collection. I have task events programmed where 1=correct, 2=incorrect, and 3=no response. While running through the task I've noticed that sometimes both a 2 and 3 appear when no response is made (but not every time). Does anyone know why this may be? Why is it inconsistent?
